Completing the Mentor/Coach Scholarship Form

• Download the IdahoSTARS Scholarship Application Form right for you.
  • If you want to work with a mentor/coach on an Individual Project, use the Training Scholarship Application Form, complete pages 1, 3 and 6. On page 3, be sure to check the box for Individual Projects. Remember to sign your application.
  • If you want to work with a mentor/coach as an Apprentice through the Apprenticeship Project, complete the Academic Scholarship Application Form, pages 1, 2 and 3. Remember to sign your application.
